Telemed Consent Form

Fill out the telemed consent form online to streamline your pet’s telemedicine visit.

Since Telemed appointments are less thorough than an in-person examination, your doctor may decide, during the consult, that your pet needs an in-person physical examination. In that case, the cost of the telemedicine consult will be applied to the in-person exam fee.


Veterinarians are limited to the amount and type of presciption medications that can be prescribed based solely on a diagnosis made by telemed examinations. As Per AZ Law (A.R.S. § 32-2240.0): Initial prescriptions based only on telemedicine may be for only a 14 day supply. The prescription may be renewed for another 14 days, with another telemed appointment. Further Renewals require an in-clinic visit. Controlled substances cannot be prescribed without an in-clinic patient exam. Further Renewals require an in-person visit. Some medications may be available at a human pharmacy. Controlled substances cannot be prescribed via Telemedcine consultations.


There may be other Pet health care team members present during the consultation other than your pet's veterinarian. If you are uncomfortable at any time, you may ask non-medical personnel to leave the telemedicine examination room: and or terminate the consultation at any time.


There is a Telemed Consult fee due prior to the appointment. We will be reaching out to you to collect payment via credit card.


If the Scheduler has not already collected the Telemedicine consultation fee, they will collect it when the Visit is started.
